This course will provide Incident Investigation and its associated causes and consequences. We will also look at reporting and investigation requirements and steps to ensure the correct approach to managing and preventing incidents

This subject consists of 3 modules, which explore incident investigations.
- Overview of incident investigation.
- Causes and consequences of incidents.
- Reporting and investigation of workplace incidents.
Objectives
- I will learn the principles applicable in understanding and differentiating between incident ‘grades’, the purpose of investigations and the obligations of the employer and employee in the event of a workplace incident.
- I will be able to investigate the direct and indirect causes and consequences of incidents (examined through a loss causation model) and understand the ratios between minor and major reported injury incidents in the workplace (as analysed by Frank Bird).
- I will explore the four steps in the sequence of incidents, which will assist me in managing and controlling all probable or resultant losses.
- I will be able to interpret the reporting requirements and steps to follow when investigating incidents in the workplace.

This subject is aimed at Supervisors, Managers and/or Safety Officers, or any person who wants to know more about safety management in the workplace.
